Job Summary: The Low Vision Rehab Physician provides specialty low vision care to patients who have experienced a vision loss, are under the care of a primary eye care professional and who wish to improve the quality of their daily living, specifically to meet individual patient-stated goals. Services will include testing, evaluation and consultation with patients. Treatment may include spectacle correction, low vision aid and technology recommendations and low vision rehabilitation therapy.
Duties and Responsibilities:
Reviews ophthalmologic reports from referring providers.
Provides functional vision assessments, routine eye examination, including visual field tests and other testing as needed.
Keeps thorough record of vision assessment and client counseling in database.
Communicates with referring eye care physicians, including written reports following-up care.
Makes referrals to other agencies when appropriate.
Keeps current with new developments in the field of optometry, specifically low vision. Takes courses (in person or online) to further develop low vision skills.
Participates in department and team meetings.
Understands and utilizes the electronic health record system.
Maintains all records in accordance with agency and legal requirements.
Understands and complies with HIPAA and agency patient confidentiality guidelines.
Graduate of an accredited School of Optometry or Ophthalmology.
Licensed to practice in the State of Oklahoma.
CPR certification obtained within 90 days of employment and kept current throughout term of employment.
Current TB testing and throughout term of employment.
Must carry your own malpractice insurance.
Ability to demonstrate dedication to client-centered approach to low vision care.
Ability to demonstrate effective team approach to rehabilitation services.
Ability to demonstrated effective working relationships with appropriate community services.
Ability to demonstrated effective communication with staff team, clients and other providers.
